Functional Life Achievement provides Early Intervention services for children from birth to age three (0-3) who have developmental delays or disabilities, as authorized under the New York State Early Intervention Program (Title II-A of Article 25 of the Public Health Law). All services are provided in accordance with each child's Individualized Family Service Plan (IFSP) as developed through the NYC Early Intervention Program and authorized by the Early Intervention Official Designee (EIOD).

Referrals submitted through our website are forwarded to the NYC Early Intervention Program for processing. Submission of a referral does not guarantee eligibility for services. All referrals are subject to the standard EIP evaluation and eligibility process as governed by NYS Public Health Law and NYSDOH regulations.
